Biggest Loser Survey
Fill out and submit by the end of the day Sunday, July 30th and receive 10 points for your team. Two people will be randomly selected to win an additional 30 points for their team. Your name must be entered in order to award points, but names will not be used when the feedback is given to the trainers.
